Setting Concepts: The setting of Holby City is a dramatised version of real life in a hospital in England. As such, there's not much to be said on concepts such as magic or advanced technology, as there isn't much difference from reality. Holby City is an on-going series that runs paralell to reality as though to give the impression that it is real.
Except Holby City Hospital has a lot more drama and cheesy soap opera tropes than any other NHS Trust in England.
That's just about the only difference. And that difference is bad writing.
Holby City Hospital
The Holby City NHS Trust encompasses the Holby City Hospital. A main hospital, Holby City provides primary healthcare and accident and emergency services to the fictional city of Holby. The hospital is based on the real life Bristol Royal Infirmary. The TV shows of Casualty and Holby City focus on four main wards.
Emergency Department
The primary Accident and Emergency reception for Holby City, this ward is ran by Connie Beauchamp, as the clinical lead. The ED is the primary reception for emergency medical issues and patients transported by the Holby Ambulance Service. It has an ajoining Clinical Decisions Unit for triaging emergency care, also known as the Acute Admissions Ward, or AAU.
Acute Admissions Unit - AAU
The AAU is attached to the Holby Emergency Department, but is a seperate ward. Acting as a short-stay ward, patients are often transferred from the ED to undergo further stabilisation and tests, or to have emergency surgery performed. The Holby AAU is ran by Serena Campbell, who also serves as Holby's Deputy CEO.
Keller
Keller ward is a general ward. Patients may be transferred from the ED or AAU if their symptoms are not critical, or do not warrant emergency proceedures. Patients can be admitted with conditions such as cancer, or severe head trauma. Keller typically houses the Neurology department, also, but this is sometimes split with Darwin.
Darwin
Darwin ward is Holby's cardiothoracic ward, specialising in cardiovascular issues and other illnesses relating to the heart and lungs. The home of the Hertzig proceedure, a fully electronic heart replacement, Darwin is considered one of the most influential wards, often recieving a lot of attention from the Board with regards to funding. Headed by the determined and career-minded Jac Naylor, Darwin is often in the spotlight.
Personality: Adrian Fletcher, known as Fletch to just about everyone, is known as Holby's "Del Boy." Essentially, a quick-tounged, fast-thinker, with a tounge-in-cheek attitude. His attitude is described as "blokey". In a sense, that means he's brash, often speaks without thinking, but generally means well. There's a genuine warmth and caring touch to everything Fletch does and says... even if it doesn't seem that way.
Being a staff nurse in the Accident and Emergency dept. of Holby City Hospital means Fletch has seen his fair share of trauma, gore and stupidity. He's well-adapted to the care of others, and takes great pride and pleasure in being able to do so. He's incredibly personable, able to strike up a rapport with patients and doctors alike, simply by treating everyone exactly the same. He's not afraid of poking fun at someone who did something stupid, or consoling someone in pain, and his actions show that he is well-suited for his job.
He gets on well with the rest of the staff, often coming in and providing support, such as when Raf di Lucca, Chief Registrar on AAU, was going through a rough time with his wife cheating on him and falling pregnant. He always appears to be able to cheer them up and keep morale running with his own brand of cheeky humour and light-hearted jabs.
Fletch has been a nurse for about three years, being promoted from a staff position on A&E to Ward Manager on Holby's Acute Admissions Unit. There, he continues to thrive and progress, showing an incredible talent at keeping things organised, despite a few hiccups when trying to cut corners to make everything work out. AAU, being the primary ward where almost all miscellaneous patients who don't slot into the specialist wards of Darwin or Keller, is often the scene of a lot of varied incidents. Fletch takes this into his stride, and often ensures the smooth running of the wardwhen the writers don't dictate otherwise.
He didn't always start off as a nurse. He shows adaptability by moving on from his chosen job; a self-employed car mechanic. The business was starting to go under due to the recession in the UK at the time and, as father of three kids, he knew he had to provide for them. He retrained as a nurse, solely so he could make enough money to keep his family going, and being inspired by the attitude of Holby staff during the birth of his third child.
That's not to say that Fletch is perfect. Shortly after joining A&E, he started an affair with his boss, Tess Bateman. It's later revealed that the reason he did so was because he'd slowly fallen out of love with his wife, Natalie. In a turbulent storyline, Fletch finds himself rejected by both his wife and his lover. He manages to cope with this, and accepts the promotion to Ward Manager on AAU.
Afterwards, Fletch isn't seen to be as promiscuous as before, despite still being quick with a chat-up line. The death of his wife, Natalie, means he's now the sole provider for his four kids. He works hard, pulling in as many shifts on AAU as he's able to, whilst still spending time with his children. He manages to split his time between his kids and his job, being a dedicated father and nurse.
All in all, Fletch might seem brash, loud, pushy and not someone everyone wants to be around, but behind that demeanour, Adrian Fletcher has one of the biggest, caring hearts in the entire hospital. If you need help, he'll do his damnedest to help, or find someone who can.
Appearance:

Abilities: Fletch is a senior staff nurse, trained in emergency medicine and triage. He can perform a number of emergency life-saving proceedures in lieu of a doctor or surgeon to prolong life. He's an excellent manager, able to stay on-top of a busy ward.
He's also an ex-car mechanic, having made the career switch when his business was under threat.

Third Person: This wasn't one of his usual haunts in Holby, but... it had to do.
Nurse Fletcher is sat in a bar in the... Water sector, he thinks. The people were nice, friendly. It seemed like a good idea to go and try to summon a bit of Dutch courage to think this through. Magic, cities on the back of turtles, shapeshifting locals... It was enough to make him believe this was all a coma... Or he was dead.
Dead seems likely. He still hurts like hell, though.
He's mulling this over when his thoughts turn to them... To his kids. Evie, Mikey, Ella and Theo... People said time back home was stopped, but... How could he believe that? He stares into his drink moodily, eyes lidded.
After a long moment, he shifts, pulling his wallet out. He drops it in front of him, opening it, and shifting through the contents. He pulls a set of photos out, smiling slightly, sadly, as he stares at the pictures of his kids, tracing his fingers over them. Next, his eyes shift to another photo. A group photo of the folks on AAU; Raf, Serena, Harry, Mary-Claire, Ric...
He sighs, putting them away, slipping his wallet into his pocket, picking up his drink.
"'ere's to you, 'olby." He slurrs, raising the glass slightly. "I 'ope to God I see you again."
Network: So, um....
[There's a man on the feed. He's looking a bit beaten up, but... Yeah. He's smiling.]
I'm still tryin' to sort this whole "city-on-a-turtle" thing out in my head. It's kinda... a lot to take in. [He laughs.] Must've been a magic grenade. Anyway... Guess I'd better introduce myself. Adrian Fletcher, call me Fletch. I'm a nurse, so... if you need help in a medical capacity, I'm your bloke.
And, well, I can sideline as a mechanic, but you lot don't seem to have many cars around here.
